Service Teams

Hospice of Dayton programs are specifically designed for individuals with a limited life expectancy. The Hospice of Dayton team approach to care is individualized for each patient and focuses on comfort and peace of mind. Hospice care is sometimes described as "Palliative Care" and addresses physical, emotional, spiritual and social needs.

Hospice of Dayton offers the following programs to provide care for those individuals facing end-of-life issues.

Coming Home

The Hospice of Dayton Coming Home Program enables patients to remain at home, in familiar surroundings, while receiving care to manage physical, psychological and emotional concerns throughout their illness. The Hospice of Dayton team has direct contact with the primary physician to provide comprehensive coordinated services.

Comfort Care

When care cannot be managed in the home or long-term care facility, the Comfort Care program can provide short-term services for pain and symptom management. Physicians and staff provide treatment at the care center in Dayton or at other contracted hospitals.

Care Partners

Similar to our Coming Home program, the Care Partners program allows patients in nursing homes or assisted living facilities to receive specialized care and support for physical, psychological and emotional concerns throughout their illness. Working closely with each facilities staff, the care team provides a wide range of services that enhance the patient's quality of life.

Pathways of Hope

Pathways of Hope offers grief counseling for families and loved ones. Our program provides one-on-one counseling and support group sessions to help cope with the loss of someone regardless of whether the loved one received care from one of our programs.
